Society Booking Form

1. A £100 deposit is required at time of booking to confirm your Tee Time.
   Provisional bookings will not be accepted.
2. On receipt of a signed Terms and Conditions form and a non-refundable deposit, a contract will be deemed to exist between Active Luton and the client.
3. Any adjustments must be made by email to proshop@activeluton.co.uk or by telephoning the centre on 01582 413704 (option 1), along with confirmed final guest numbers 14 days prior to the event, when the full amount MUST be paid. Failure to do so will result in the full quoted charge being applied.
4. All catering requirements will be invoiced separately from Elevenses Catering. Full catering payment is required 14 days prior to the event. Please contact direct: talk2us@elevensesluton.com /01582 353993
5. In the event of Stockwood Park Golf Centre having to cancel the booking, the booking will be rescheduled where possible, or a full refund will be made.
6. Signatories on the Terms & Conditions form are authorised to act on behalf of the client as their agent.
7. Active Luton does not accept responsibility for the group’s failure to complete rounds due to poor weather conditions in the instance of the course remaining open.
8. The client will abide by Stockwood Park Golf Centres dress code policy: golf shoes to be worn on the main course. No torn or distressed denim, football / rugby or sleeveless shirts allowed.
9. The minimum number for a society booking is 12 players. Should less than the required minimum number of players arrive on the day, your group will be required to pay the standard green fees and outstanding balance. If you wish to receive a reminder phone call for payment please tick here.

10. By signing this form I confirm I have read and understood the terms and conditions of the booking and I agree to abide by them.
